The speaker from the 2-day Aviation Human Factors PDP on February 2-3 will use this workshop on a smaller scale to illustrate that a review of aircraft accidents commonly reveals that a series of events were allowed to develop well before the actual accident. If aviation personnel can break that “chain-of-events,” accident occurrences become less likely.
