CAM Exam Accommodation Request Form

I understand that I may be required to submit additional information to NBAA staff to review the requested accommodation. I understand and agree that NBAA staff may provide my records to an appropriate professional selected by NBAA for an independent evaluation relating to my request. I give my permission for my diagnosing professional to discuss with NBAA staff my records and history as much as they relate to the requested or suggested accommodation. I understand that if I choose to provide existing documentation of the same or a similar accommodation, I may be required to provide additional verification.
